Sciatica vs Lower Back Pain: How Can You Tell the Difference and What Treatments Can Help

Aching in your lower back does not automatically mean you have sciatica. The clue is often what happens next: Does the pain stay in your back, or does it travel into your buttock, thigh, calf or foot?

That difference matters because sciatica involves irritation or compression of nerves in the lower spine, while many cases of lower back pain are not caused by nerve compression at all. The treatment approach can therefore be very different.

Lower back pain is extremely common. The World Health Organization estimates that 619 million people worldwide experienced low back pain in 2020, with that number projected to reach 843 million by 2050. 

About 90% of low back pain is classified as nonspecific, meaning a single structural cause cannot always be identified.

So, how can you tell whether your symptoms are more consistent with sciatica or localized lower back pain? 

And when should you consider professional evaluation for sciatica treatment?

Sciatica and Lower Back Pain Are Not the Same Thing

“Lower back pain” is a broad description. It can involve muscles, ligaments, joints, discs or other structures around the lumbar spine.

Sciatica is more specific. It describes a pattern of pain caused by irritation or compression involving the nerve roots that contribute to the sciatic nerve. A herniated disc is one possible cause, but spinal narrowing and other conditions can also contribute.

The important point is that pain location alone cannot establish a diagnosis. A person can have both lower back pain and nerve-related leg pain at the same time.

The simplest distinction: where does the pain travel?

These patterns are useful clues, but they are not a substitute for an examination. Even conditions that look similar can require different treatment.

What Does Sciatica Usually Feel Like?

Sciatica often has a recognizable “traveling” quality.

You may notice pain that begins around the lower back or buttock and extends into the thigh, calf or foot. It may feel sharp, burning, shooting or similar to an electric shock. Numbness, tingling or weakness can also occur.

For some people, the leg pain is actually more prominent than the back pain.

A herniated lumbar disc is a common cause. If disc material irritates a nearby nerve, symptoms can develop along the pathway supplied by that nerve. 

The American Association of Neurological Surgeons notes that lumbar nerve irritation can produce radiating pain, burning, tingling or numbness and may affect one side of the body.

However, not every pain that travels into the leg is automatically sciatica. Other nerves, joints and soft tissues can produce similar symptoms. That is one reason an accurate assessment matters.

What Does Lower Back Pain Usually Feel Like?

Lower back pain can be much less predictable.

You might experience:

  • A dull or aching sensation
  • Stiffness after sitting or sleeping
  • Pain when bending, twisting or lifting
  • Muscle tightness or spasms
  • Pain concentrated around the lumbar region
  • Discomfort that changes with posture or activity

There are many potential sources, including muscles, spinal joints and discs. In many cases, no single structural abnormality can explain the symptoms.

This is one reason imaging findings need to be interpreted carefully. A scan can show disc degeneration or other changes without proving that the finding is responsible for someone’s pain.

For uncomplicated low back pain, routine imaging is generally not recommended in nonspecialist settings unless there is a clinical reason to suspect a more serious or specific condition.

Why Can Sciatica and Back Pain Feel So Similar?

The lumbar spine is a relatively small area containing many different structures. Pain from one structure can also influence how nearby muscles and nerves respond.

For example, a person with a lumbar disc problem may experience both:

  • Localized lower back pain, and
  • Radiating leg pain caused by nerve irritation.

That means asking, “Is it sciatica or back pain?” can sometimes be the wrong question.

A more useful question is:

“What is causing my back and/or leg symptoms?”

That shift is important because sciatica treatment should be based on the underlying cause, symptom pattern, neurological findings, health history and functional limitations rather than the label alone.

What Can Cause Sciatica?

Several conditions can irritate or compress nerves in the lower spine.

  • Herniated disc

A lumbar disc can develop a tear or protrusion that irritates a nearby nerve. This is one of the best-known causes of sciatica.

  • Spinal stenosis

Narrowing of the spaces around the spinal nerves can occur as the spine changes with age. Depending on the location and severity, this can contribute to leg pain, numbness or weakness.

  • Degenerative changes

Changes involving discs or spinal joints may sometimes contribute to nerve irritation. Importantly, degenerative findings on imaging are common and do not always cause symptoms.

  • Other causes

Less common conditions can also produce symptoms that resemble sciatica. A clinician may consider the complete symptom pattern, physical examination and medical history when determining what should be investigated.

What Can Cause Lower Back Pain?

Lower back pain has an even broader list of potential contributors.

These may include:

  • Muscle or ligament strain
  • Spinal joint irritation
  • Disc-related pain
  • Degenerative changes
  • Reduced mobility or deconditioning
  • Repetitive physical demands
  • Certain inflammatory or systemic conditions

In many cases, the pain is described as nonspecific low back pain, meaning there is no single identifiable structural cause.

That does not mean the pain is imaginary or unimportant. It means treatment often focuses on improving movement, function and pain-related limitations rather than correcting one specific abnormality.

What Is the Right Treatment for Sciatica?

There is no single “best” sciatica treatment for every patient.

Treatment depends on the cause, severity, duration of symptoms, neurological findings and response to initial care.

1. Activity modification and rehabilitation

For many people, conservative care is an appropriate starting point.

Depending on the clinical situation, this may include maintaining appropriate activity, avoiding movements that significantly aggravate symptoms and using a structured rehabilitation program.

Prolonged bed rest is generally not recommended for uncomplicated disc-related sciatica. Walking as tolerated and physical therapy may be part of nonsurgical management.

2. Medications

A clinician may recommend medication based on the individual’s medical history, other medications and potential risks.

Common options can include anti-inflammatory medicines or other prescription treatments when appropriate.

Medication should be selected with a healthcare professional, particularly for people with kidney disease, gastrointestinal problems, cardiovascular conditions, medication interactions or other health concerns.

3. Physical therapy

A targeted rehabilitation program may address mobility, strength, movement patterns and functional limitations.

The goal is not simply to stretch the painful area. A physical therapist can help determine which movements are appropriate and progressively build tolerance.

4. Epidural steroid injections

For selected patients with acute, severe sciatica, an epidural injection containing local anesthetic and corticosteroid may be considered.

Consider epidural injections for acute and severe sciatica, while emphasizing that treatment decisions should be based on the clinical situation.

An injection is not appropriate for every person with back pain, and it should not be viewed as a universal solution. The potential benefits and risks need to be considered for the individual.

5. Surgery in selected cases

Most people with a herniated disc and associated sciatica do not require surgery. However, surgical evaluation may be appropriate when symptoms persist despite appropriate nonsurgical treatment, when neurological problems are progressing, or when imaging findings correspond with significant symptoms.

Consider spinal decompression when nonsurgical treatment has not improved pain or function and radiological findings are consistent with the person’s sciatica.

What Is the Best Lower Back Pain Treatment?

The answer depends heavily on why the back hurts.

For uncomplicated lower back pain, treatment commonly emphasizes conservative measures rather than immediately pursuing invasive procedures.

  • Exercise and rehabilitation

A structured exercise program can help improve mobility, strength and physical function.

For chronic low back pain, the World Health Organization recommends a range of interventions that can include education, exercise and other forms of rehabilitation, depending on the individual and care setting.

  • Activity and movement

Avoiding every activity because of fear of worsening the spine can sometimes make returning to normal function more difficult.

The appropriate level of activity depends on the cause and severity of the pain. A clinician or physical therapist can help determine what is reasonable.

  • Medication when appropriate

Medication may sometimes be used as part of a broader treatment plan. The choice should consider the person’s medical history, other medications and potential adverse effects.

  • Targeted procedures for selected pain sources

Not every type of lower back pain responds to the same procedure.

For example, if a clinician determines that facet joints are contributing to chronic localized back pain, diagnostic injections or radiofrequency-based treatment may sometimes be considered in appropriately selected patients.

The key is targeted treatment rather than treating every case of back pain as though it has the same source.

When Should You Get Your Back Pain Evaluated?

You do not necessarily need advanced imaging or an invasive treatment the moment your back starts hurting.

However, an evaluation becomes especially important when symptoms are persistent, recurrent, severe, interfering with normal activities or accompanied by neurological changes.

You should seek prompt medical attention for symptoms such as:

  • New or worsening significant leg weakness
  • Loss of sensation around the groin, genitals or buttocks
  • New difficulty controlling urination or bowel movements
  • Severe or rapidly worsening neurological symptoms
  • Back pain following significant trauma
  • Back pain accompanied by concerning systemic symptoms such as fever

These symptoms can indicate a condition requiring urgent assessment. For example, urinary retention, bowel or bladder dysfunction and saddle-area numbness are recognized warning signs of cauda equina syndrome, a medical emergency.

Does Everyone With Sciatica Need an MRI?

No.

An MRI can be valuable when the results are likely to change management, but imaging is not automatically necessary for every episode of back pain or sciatica.

A scan can also create confusion because structural changes may exist without causing symptoms.

The decision to order imaging should take into account the duration and severity of symptoms, neurological findings, suspected cause and whether the results would influence treatment.  advises against routine imaging for low back pain with or without sciatica in nonspecialist settings.

Sciatica vs. Lower Back Pain: The Takeaway

The most useful distinction is not simply whether your back hurts.

Localized aching or stiffness that stays primarily in the lower back is more consistent with many forms of mechanical or nonspecific lower back pain. 

Pain that travels from the lower back or buttock into the leg, particularly when accompanied by tingling, numbness or weakness, raises more concern for nerve-related pain such as sciatica.

But symptoms overlap, and self-diagnosis has limits.

The right treatment depends on identifying what is actually driving the pain. For some people, rehabilitation and activity modification may be enough. 

Others may need medication, targeted injections or, in selected cases, surgical evaluation.
